  • Flu prevention

    We recommend that everyone who is six months old or older should get a flu shot. This is important for people who are 65 and older, pregnant women, young children, and people with long-term health issues like heart, lung, or kidney disease. Flu shots are available at no cost to members when received at an in-network pharmacy. Log in to your My Collective® account and use the Get Care search to find and in-network pharmacy.

  • Quit smoking

    Quitting tobacco takes commitment and can be tough if you're doing it alone. Research shows that combining counseling or support with medications like nicotine replacements or prescriptions gives you the best chance to quit successfully.


    Whether you’ve been smoking for six months or 16 years, we're here to support you. Find what works best for you, whether it's support groups, phone counseling, or classes with free medications. Providence Health Plan Powered by Collective Health provides you with full support to quit tobacco at no cost to you.


    Program benefits:


    • Quit Coach consultations: Get help from a coach who understands quitting challenges and can help keep you on track.
    • Personalized Quit Plan materials: Resources to help you plan and stick to your personalized Quit for Life program.
    • Nicotine replacement therapy: Access to various nicotine replacement options.
    • Coverage for prescribed medications: Certain prescriptions are covered to help you quit.

    Connect with a coach over the phone or through live chat to create a personalized quit plan and receive support at every step. You'll also have access to resources to manage triggers and cravings.

    Important note:


    Tobacco users have $25 deducted monthly from their pay due to increased healthcare costs to their health plan. You can stop this deduction after quitting by completing a Mid-year Change form.


    Tobacco users pay higher rates for optional life insurance. You can qualify for a lower rate if you’ve been tobacco-free for 12 months before the next open enrollment.

  • Diabetes prevention and management

    Preventing and managing diabetes is important for your health. Caring for diabetes can stop problems like heart disease, nerve damage, and losing your vision. Our programs give you the help and tools you need to stay in control of your health:
