You'll also have access to the highest quality healthcare in the state of Oregon, through OHSU and Providence hospitals. To find in-network care near you, log in to your My Collective® account and use the Get Care provider search.

What you pay for certain services

Medical
Deductible (in-network) $250 individual; $750 family
Out-of-pocket maximum $1,900 individual; $5,700 family
Primary care 15% or 10% coinsurance for the first four visits per calendar year ($0 deductible)
Specialty care 15% coinsurance
Preventive services (in-network) No cost ($0 deductible)
Virtual visits (in-network) No cost ($0 deductible)
Massage therapy, chiropractic, and acupuncture 15% coinsurance (deductible waived)
Prescription Drugs
Deductible $50 per person; $150 family
Out-of-pocket maximum $1,000 per person; $3000 family
Value drugs $0 copay
Generic drugs $10 copay
Specialty drugs $100 copay
90-day supplies Copay x2.5

  • Plan features
    • Full access to the nationwide Providence Signature Network, with over one million providers, including OHSU and Providence hospitals
    • No specialist referral requirements
    • Wellness resources to help with chronic disease, stress, diet and exercise, sleep, and more
    • Coverage for medically necessary services when traveling internationally
    • Any amount paid toward your Providence plan deductible in October, November, and/or December 2025 is applied to your 2026 Providence plan deductible

  • Selecting a Primary Care Provider (required)

    Members and their eligible dependents enrolled in the Providence Statewide plan are required to select a primary care provider. To make your selection:


    • Log in to your My Collective® account.
    • Navigate to the Get Care provider search and choose a provider.
    • Click ‘Set as my primary care provider.’
