With a Providence Health Assurance medical plan, you have access to the following free resources:

Care management

Healthcare can be confusing. Providence Health Assurance Care Management can help you:

  • Find a doctor or provider for your care needs
  • Schedule a ride to see your doctor
  • Answer questions related to your diagnosis and plan of care
  • Understand the health care system, for example: Who to see, what hospital to choose, coordinating care when you see more than one doctor, and health education
  • Set up a care plan for conditions such as Diabetes, Pain Management, Cancer, Pregnancy, or support to caregivers of children who need help with an illness

Flexible Services

Sometimes you may have needs that are not covered by your Medicaid benefits. Providence Health Assurance has a fund that the Oregon Health Plan calls Flexible Services. Flexible Services are items like temporary lodging, food, and household essentials, as well as cell phones which allow you to stay connected to your care team and providers. These items can be key to your health and may be covered under this fund. Any health care provider can make a request for you. If you think you need access to Flexible Services, please talk to your Primary Care Provider (PCP).

Traditional Health Workers

There may be times when you need help getting the right care. Traditional Health Workers are specially trained to help you.


THWs can be found in both clinic and community-based settings. They are a free benefit to members and do not require a referral or prior authorization. Any member can access a THW.


Oregon recognizes 5 types of THWs:


  • Birth Doulas
    Birth Doulas are trained, certified birth companions who provide personal, non-medical support to people and their families during pregnancy, childbirth, and postpartum.
    Some of Health Share’s Birth Doulas are also trained as Peer Support Specialists, allowing them to provide specialty care to pregnant and postpartum people who are experiencing substance use conditions. For more information on how to access this care, see Project Nurture webpage.
  • Community Health Worker
    Community Health Workers are trusted, frontline public health workers who are members of and/or have close relationships with the communities they serve. CHWs bridge community with health systems and social services to improve access to care. CHWs can help by delivering health education, providing health screenings, providing culturally appropriate health information, and connecting members to resources and services.
  • Personal Health Navigator
    Personal Health Navigators are trained, certified individuals who provide information, assistance, tools, and support to help people make health care decisions that are aligned with the person’s needs, lifestyle, conditions, and desired outcomes.
  • Peer Support Specialist
    Peer Support Specialists are trained, certified individuals who provide services and supports to people currently experiencing mental health conditions and/or addiction. They have shared life experiences, such as experience with substance use, mental health conditions, or having family members with mental health conditions and/or addictions.
    Some Peer Support Specialists are also trained as Birth Doulas, allowing them to provide specialty care to people experiencing substance use conditions while also pregnant and postpartum. For more information on how to access this care, see Project Nurture webpage.
  • Peer Wellness Specialist
    Peer Wellness Specialists are individuals with training and personal experience with psychiatric conditions. They work as part of person-driven home health teams, combining mental/behavioral health with primary care to assist and advocate for individuals in achieving well-being.


Accessing care:


Health Share/Providence members have access to CHWs, Birth Doulas, and Lactation support through their primary care provider (PCP). You can get connected to a THW through your primary care doctor, care manager, and you can contact customer service to learn more (800-898-8174 TTY: 711).

Quitting tobacco

Quitting takes practice, patience and support. Providence is here to help you.

  • Online program - The American Lung Association of Oregon's Freedom From Smoking® program offers a lot of options to help you quit smoking. To learn more about the online program, please visit the Freedom From Smoking® Online website.
  • Telephone program - Quit for Life is designed for smokers who want a structured program but cannot attend a regular class. To register, please call 866-QUIT-4LIFE (866-784-8454), TTY: 711. This program is free for Providence Health Assurance members.


Ride to Care

Do you need a ride to your doctor appointment? Ride to Care provides free, non-urgent rides for covered appointments. They can also help you with other items like bus tickets or mileage reimbursement. You can call them at 1-855-321-4899, TTY:711, Monday through Friday, 8 a.m. to 5 p.m. Your Health Share of Oregon member handbook can tell you more about this. You can also learn more online.



Health and exercise classes

As a Providence Health Assurance member, you can take Providence health and wellness classes – many at a discount. Take a fitness class, like Zumba or yoga, or learn more about a health issue.
