What is Health Share of Oregon?
Health Share of Oregon is a coordinated care organization (CCO) that serves Oregon Health Plan (Medicaid) members in Clackamas, Multnomah and Washington counties. It connects you to the local services you need to be healthy.
Health Share of OregonWith Providence Health Assurance as your physical health plan:
- You are a Health Share/Providence member.
- You will use your Health Share of Oregon member ID card. It has your physical, dental and mental health providers’ information printed on the back.
- Health Share of Oregon will mail you a member handbook that explains your benefits and how they work.
- You will use our network of providers to choose your primary care provider.

Physical health
Providence Health Assurance manages your primary care, most specialty care, most hospital services, urgent and emergency care, and most prescription drugs.

Mental health and substance use
Health Share's mental health and substance use provider network is managed by CareOregon.
To get care and services:
- Find a Mental Health or Substance Use Provider online
- Call CareOregon at 503-416-4100
- Talk to your primary care provider (your main doctor or clinic) listed on your ID card
For urgent mental health support call your county's mental health crisis line 24/7. Clackamas County: 503-655-8585. Multnomah County: 503-988-4888. Washington County: 503-291-9111.

Health Related Social Needs (HRSN) Benefits

- Housing support: Help you stay in your home.
- Nutritional support: Help with food benefits and eating healthy.

Traditional Health Workers
There may be times when you need help getting the right care. Traditional Health Workers are specially trained to help you.
THWs can be found in both clinic and community-based settings. They are a free benefit to members and do not require a referral or prior authorization. Any member can access a THW.
Oregon recognizes 5 types of THWs:
- Birth Doulas
Birth Doulas are trained, certified birth companions who provide personal, non-medical support to people and their families during pregnancy, childbirth, and postpartum.
Some of Health Share’s Birth Doulas are also trained as Peer Support Specialists, allowing them to provide specialty care to pregnant and postpartum people who are experiencing substance use conditions. Community Health Workers are trusted, frontline public health workers who are members of and/or have close relationships with the communities they serve. CHWs bridge community with health systems and social services to improve access to care. CHWs can help by delivering health education, providing health screenings, providing culturally appropriate health information, and connecting members to resources and services. - Personal Health Navigator
Personal Health Navigators are trained, certified individuals who provide information, assistance, tools, and support to help people make health care decisions that are aligned with the person’s needs, lifestyle, conditions, and desired outcomes. - Peer Support Specialist
Peer Support Specialists are trained, certified individuals who provide services and supports to people currently experiencing mental health conditions and/or addiction. They have shared life experiences, such as experience with substance use, mental health conditions, or having family members with mental health conditions and/or addictions.
Some Peer Support Specialists are also trained as Birth Doulas, allowing them to provide specialty care to people experiencing substance use conditions while also pregnant and postpartum. For more information on how to access this care, see Project Nurture webpage. - Peer Wellness Specialist
Peer Wellness Specialists are individuals with training and personal experience with psychiatric conditions. They work as part of person-driven home health teams, combining mental/behavioral health with primary care to assist and advocate for individuals in achieving well-being.
Accessing care:
Health Share/Providence members have access to CHWs, Birth Doulas, and Lactation support through their primary care provider (PCP). You can get connected to a THW through your primary care doctor, care manager, and you can contact customer service to learn more (800-898-8174 TTY: 711).
